You can pay for your gift certificate:

1. Complete the lower portion of the gift certificate and fax with credit card information, expiration date, and signature to MBS Fitness at 323.464.8789.

          or

2.  Complete the lower portion of the gift certificate and mail with check to MBS Fitness, 15175 Magnolia Blvd, Los Angeles, CA 91403.

After we receive your payment, we will contact the recipient of your gift and give them credit for the appropriate MBS Fitness program.
